By signing this application, i declare:

1) that applicant is aware of the certification rules and accept to at all times comply with the criteria and procedures hereof cf.:

       a) Declaration and other normative documents
       b) Certification requirements for standard owner’s welfare mark
       c) Application and certification by Baltic Control Certification A/S 

2) that applicant is aware of the implications of the certification with reference to standard owner’s welfare mark
 

3) that applicant will provide all necessary information and make documents available in order for Baltic Control Certification A/S to assess the application.
 

4) Baltic Control Certification A/S reserves the right to forward any relevant information of importance to the certification to standard owner.
 

5) that applicant provide the Baltic Control Certification A/S assessment team access to all relevant localities and on request arrange for the team to monitor the certification applicant carrying out the practicalities of production, and that Baltic Control Certification A/S be allowed to document any non-conformities by means of photography. If the assessment team is refused the opportunity to take photographs, the applicant may not be approved.
 

6) that the Baltic Control Certification A/S assessment team will be granted access to all personnel employed within the product areas applied for
 

7) that applicant will only claim certification within the area for which the certification is granted. That applicant will not utilise the product certfication in any way or form that may bring Baltic Control Certification A/S into disrepute and will not make any claims regarding the applicant’s certification which Baltic Control Certification A/S may consider to be misleading or unauthorised.
 

8) that applicant will only use the certification to demonstrate that certified products comply with specified standards.
 

9) that applicant ensures that certificates or reports or parts hereof will not be used in any misleading way. And that requirements of Baltic Control Certification A/S for referring to the product certification in the media, documents, pamphlets or advertising are complied with.
 

10) that in the event of suspension or withdrawal of the certification, any use of advertising material referring to the certification will be put to a stop, and all certification documents requested by Baltic Control Certification be returned.
 

11) that applicant discloses all changes that relate to the product certification, such as intent to change the product, the manufacturing process or the quality management system, having an impact on the compliance of the product. Baltic Control Certification A/S will determine whether the disclosed changes require further examination. If so, applicant must refrain from releasing any certified products manufactured after having implemented such changes until Baltic Control Certification A/S has informed applicant of the result.
 

12) that Baltic Control Certification A/S may make use of sub-contractors.
 

13) that applicant will pay any fees according to the at any time valid product certification price list of Baltic Control Certification A/S.
 

14) that Baltic Control Certification A/S may store your contact information. Your data is stored in compliance with the regulations of the Danish Data Protection Act.
 

15) Below livestock company declares as a sworn statement that the livestock company completes a documented self-regulation according to standard owner’s concept, and that Baltic Control Certification A/S and standard owner will be informed in the event of any violations of the concept.

16) that Baltic Control Certification A/S confidentially process all data which are received before, during and after audits.

Furthermore, the company accepts disclosure of control data to third party.
